1. Acceptance of Terms:
By accessing and using the Quezon Provincial Hospital Network - Bondoc Peninsula (Catanauan) Hospital Information System (hereinafter referred to as "the System"), you acknowledge that you have read, understood, and agree to be bound by these Terms of Service (hereinafter referred to as "Terms"). 2. Purpose of the System:

Prohibited Activities: Users must not engage in any activities that could compromise the security or integrity of the System, including but not limited to:
Unauthorized access to data.
Distribution of malware or viruses.
Attempting to circumvent security measures.
Misuse of patient information.
Any activity that violates local, regional, or national laws.
4. Patient Information and Privacy:
Confidentiality: Patient information stored in the System is confidential and protected by applicable privacy laws, including the Philippine Data Privacy Act of 2012.
Authorized Access: Access to patient information is restricted to authorized personnel who require such access for legitimate healthcare or administrative purposes.
Data Security: The hospital implements appropriate security measures to protect patient information from unauthorized access, use, or disclosure.
Patient Portal (if applicable): If a patient portal is provided, patients are responsible for maintaining the confidentiality of their portal login credentials and for using the portal responsibly.
5. System Availability and Maintenance:
Availability: The hospital will make reasonable efforts to ensure the availability of the System, but does not guarantee uninterrupted access.
Maintenance: The hospital may perform scheduled or unscheduled maintenance on the System, which may result in temporary service interruptions.
System Updates: The hospital reserves the right to update or modify the System at any time.
6. Limitation of Liability:
The hospital shall not be liable for any direct, indirect, incidental, consequential, or punitive damages arising from the use or inability to use the
System.
The hospital shall not be liable for any errors or omissions in the information contained in the System.
The hospital is not responsible for data loss due to events outside of reasonable control, such as natural disasters, or power outages.
7. Intellectual Property:
The System and its content, including software, databases, and documentation, are the property of Quezon Provincial Hospital Network - Bondoc Peninsula (Catanauan) or its licensors and are protected by intellectual property laws.
Users are prohibited from copying, distributing, or modifying the System or its content without the express written permission of the hospital.
8. Governing Law and Jurisdiction:
These Terms shall be governed by and construed in accordance with the laws of the Republic
of the Philippines.
Any disputes arising from these Terms shall be subject to the exclusive jurisdiction of the
courts of competent jurisdiction in the Philippines.
9. Changes to Terms:
The hospital reserves the right to modify these Terms at any time.
Users will be notified of any material changes to these Terms.
Continued use of the System after such changes constitutes acceptance of the revised Terms.
